Go to your Cash App home screen
Tap on the tab that shows ‘Cash Card’
Tap on the tab that reads ‘Activate Cash Card’
Your Cash App will ask for permission to open the camera, once you see it, tap on ‘Ok’
Focus the camera on the QR Code

In case, you do not have the QR Code any longer follow the given steps:

Tap on the tab that reads ‘Cash app’
Tap to ‘Activate’
Then, Tap on ‘Help’
Select the tab ‘Use CVV Instead’
Enter the ‘CVV code’ and the new Cash Card’s expiration date
